The Health Program Coordinator will be responsible for day-to-day development, coordination, implementation and evaluation of the organization’s various health programs and initiatives relating to the promotion of preventative health care, family health care, HIV/AIDS prevention and HIV rapid testing, and linkage of recent immigrants to culturally sensitive health, mental health and social services.
Responsibilities:
Develop formal client intake interview assessment and quarterly follow-up tools;
Conduct intake interviews and follow-up assessments with clients;
Develop and implement case management plans;
Train and supervise the Community Health Promoters to address a broader range of health issues that affect the community;
Train and supervise the Community Health Promoters to conduct client intake assessment interviews and make appropriate service referrals;
Formalize and implement a computer-based client tracking system that will enhance the organization’s ability to carry out and follow-up on referral for benefits and other services;
Increase the number of and diversify of linkages with culturally sensitive health, mental health and social service providers in the community and citywide;
Facilitate individual and group-level health promotion interventions;
Provide HIV rapid counseling and testing;
Co-supervise agency’s HIV rapid counseling and testing program;
Work with the Education Coordinator and volunteer instructors to integrate health education into adult education and after school youth program curricula;
Represent the organization at community-based and city-wide networking meetings and committees;
Aid in developing marketing and advertising tools relating to preventative health education and health services;
Complete monthly reports and reports to funders, as needed; and
Assist Executive Director in fundraising to support health programs.
Req. candidate profile :
Qualifications include:
MSW, or Masters degree and several years professional experience in psychology, mental health, social work, public health or a related field;
Bi-lingual in English and Spanish;
Knowledge of Mexican and Central American culture and issues affecting recent Latino immigrants
Knowledge of government benefits and health, social services and public education systems and resources;
Strong English and Spanish verbal and written communication skills;
Strong organizational skills and excellent follow-up;
Able to work independently and as a member of a team;
Computer literate (knowledge of Microsoft Access a plus);
Able to work in a fast-paced, grass-roots community organization;
