Job description
Job description: Duties and Responsibilities:
  • Carry a caseload of Asian developmentally disabled (DD) clients, and provide case management services through home visits, service coordination and purchase of service order preparation;
  • Conduct social assessment and IFSP primarily for the Early Intervention children ages 0-3 with at risk conditions; and all the reviews and IPP for all DD cases.
  • Write up all case documents in a timely manner as required by RCEB/ DDS.
  • Act as an advocate for the clients and families for services in the mainstream system.
  • Carry a caseload of Asian families with multiple issues, and perform a variety of direct clinical, diagnostic, treatment, and case management services through home visits;
  • Conduct diagnostic assessments primarily for the children ages 0 – 5 with beh. and/or MH issues;
  • Participate in RCEB case related meetings, BHCS utilization review, and agency training and meetings.
  • Provide DD/MH consultation, education, information and referral for other programs or community fair as requested;
  • Other duties as may be required by the Director of Family and Disabilities Services.
Req. candidate profile : Additional Qualifications:
  • Master degree in Social Work or related field (preferred) or bachelor’s degree plus work/internship experience;
  • Bicultural/bilingual in Vietnamese required;
  • Minimum one year of work experience with DD clients or Asian clients with mental health issues; and knowledge of clinical issues with family with children age 0-5 is a plus;
  • Ability to work independently and to exercise professional discretion and judgment;
  • Ability to work with a culturally diverse staff and client population;
  • Commitment to working in the Asian community;
  • Valid California driver’s license, auto insurance with bodily injury liability limit of at least $100,000/$300,000 and access to use of an automobile;
  • Support and implement the philosophy, goals, and objectives of Asian Community Mental Health Services.
