Homes for the Homeless (HFH) is the largest single provider of transitional housing in New York City. At our five American Family Inns, families receive the education, job training, and support services necessary to maintain permanent housing and become self-sufficient. Our shelter in the JFK area of Queens seeks a Case Manager with BA or BS and previous experience to work with our diverse population.
Duties and Responsibilities:
Effectively interview participants as to their needs, goals, interest and barriers.
Develop Service Plans based on assessment and in conjunction with participants goals and specific activities.
Assists participants with referrals and monitoring follow-up.
Reassesses participants skills, needs, goals, interest and barriers as needed.
Maintain and update participants files.
Monitor and document participants progress towards their goals.
Req. candidate profile :
Additional Qualifications:
Bachelor's Degree
Strong written and oral communication skills.
Exceptional organizational skills and strong attention to details.
Excellent inter-personal skills.
Excellent skills in Microsoft Office - Excel, Access, Outlook, and PowerPoint.
Proficiency in English and Spanish is highly preferred
