Summary of Responsibilities:
The Social Worker / Case Manager, under the supervision of the manager of Senior Services Programs, will assist senior citizens to enhance the quality of their lives through integrated care management, services including information and referral, case management, and advocacy, so they can maintain their independence and live at home. He/she will be assigned to work with seniors in the Little Tokyo area and in the South Bay.
The Social Worker / Case Manager must be able to work with senior clients in an empathetic and caring manner. He/she must possess a wide range of skills and attributes, and must be willing to learn. He/she must be able to work with a diverse staff and programs, as well as be flexible and take initiative when appropriate.
Duties and Responsibilities:
- Conduct intake sessions, assess client needs, develop case plans and coordinate services according to clients’ needs;
- Maintain case notes and other documentation current;
- Provide progress reports and performance goals as required by the service contracts and/or by the supervisor;
- Ensure all required reports, including financial reports, are submitted in a timely manner;
- Make referrals to appropriate resources for mental health assessment and counseling;
- Attend all required or appropriate meetings, including Social Service staff meeting;
- Conduct workshops or educational seminars to groups of seniors as necessary;
- Maintain and be familiar with resources and agencies that assist seniors;
- Work cooperatively with other agencies and organizations that benefit seniors;
- Attend trainings, seminars and conferences as assigned;
- Fulfill other duties as deemed necessary and appropriate, including activities and assignments relating to the Little Tokyo Service Center as a whole.

Qualifications and Experiences:
- Bachelor’s degree in Social Work, Sociology, Gerontology or related field, OR one year of experience in social service, community or nonprofit agency;
- Basic computer literacy and ability to write;
- Bilingual in English and Japanese or English and Korean preferred;
- California driver license and access to an insured automobile.
