Community Healthcare Network (CHN) is a not-for-profit organization that provides comprehensive community-based primary care, mental health care, and social services for low-income individuals in ethnically diverse, underserved communities throughout New York City.
CHN is currently recruiting for a Program Manager for its Disease Management Program. The Program Manager will be responsible for managing CHN'S Diabetes Care and Education program by providing group and individual self-management education, and coordination and evaluation of diabetes clinical care. The Program Manager works as part of the healthcare team at CHN's Bronx Health Center and is also a part of the Health Education Department.
Additional Responsibilites include but not limited to:
Promoting community awareness of diabetes by identifying and attending community events, health fairs, and marketing opportunities, as appropriate. Coordination of training for CHN staff on diabetes, self-management, and other skills related to diabetes care.

Additional Qualifications:
Bachelor's degree with applicable community health experience. Master's degree preferred, but not required. Bilingual Spanish speaking required. General knowledge and understanding of diabetes, diabetes care, and diabetes education. Demonstrated ability to organize , develop, implement, and evaluate health education programs.
